How To Join
The SGMP membership applications are available
below as PDF (Adobe Acrobat) files. You may download the membership
application appropriate for your membership, and then print it locally from
your computer. You need to have Acrobat Reader to view and print this file.
This software is available free of charge. Links below are to the forms
effective October 1, 2010.
Government Planner and Contract Planner Application (30 year anniversary special)
Supplier and Associate Supplier Membership Application
Educator/Student Membership Application and Retiree Transfer Form
Complete the printed application. Planners AND CONTRACT PLANNERS must also complete and return the planner profile with the application. MAIL your ORIGINAL SIGNED AND DATED application, along with your membership fee payable to SGMP, to the membership contact listed at the top of this page.
Faxed or emailed applications WILL NOT BE ACCEPTED and will delay the processing or your membership!
Note: Supplier applications must be submitted with a completed planner application or they will be returned to the applicant.
