Changes in name, address, and telephone number must be submitted in writing to the SGMP Membership Office. Print or download the Name Change Form below and complete. Please print or type! Be sure to sign your form! It will not be processed unless it is signed. Note: Effective March 17, 2001 there is no longer any fee assessed for submitting a name change.
